The social determinants of health play an important role in shaping the health and well-being of individuals across the lifespan. As with all health assessment, it is imperative for health care professionals to strive to keep an open mind, respect differences, and be aware of personal implicit biases as they assess for social, cultural, economic, educational, and environmental factors influencing health.

Reflect upon the learning accomplished during Module One regarding these variables, and respond to the following:

Identify three of the social determinants of health and explain the impact of each determinant on individual health and person-centered care.

Using a holistic approach to assessment, what strategies would you use to determine how the social determinates influence the individual's health? Provide three questions that might aid in assessing these variables.

How might the Synergy Model provide a framework for the MSN-prepared nurse to collaborate with individuals in order to assess health literacy and the impact of social determinants?

During the Module Three learning activities, the role of the MSN-prepared nurse as an educator is evident. Whether providing education to individuals, families, or groups, improving the knowledge surrounding health and illness is an integral component of advanced nursing practice, with health promotion often being the goal.

With learning from Module Three regarding the respiratory system in mind, as well as concepts surrounding health promotion from the Module Two lesson, respond to the following:

Describe a scenario involving either a chronic or acute illness involving the respiratory system that is different from the Shadow Health patient scenario.

Across healthcare settings, an increased incidence of complex disease and disability creates unique needs for care delivery. The advanced practice nurse plays a critical role in optimizing health outcomes for individuals with complex health concerns through holistic, interdisciplinary approaches to care. Through collaborative practice with interprofessional colleagues and community partners, strategies can be implemented to support the health and well-being of individuals with complex health concerns.

With learning from Module Five regarding the musculoskeletal system in mind, respond to the following:

Describe a scenario including an individual with a disease, genetic disorder, or disability involving the musculoskeletal system.

Discuss how the MSN-prepared nurse can collaborate with interprofessional colleagues to optimize health outcomes. Provide an example that applies to your scenario.

Explain how competency in advanced physical assessment, pathophysiology and pharmacology contributes to interprofessional collaborative practice. Provide an example to illustrate the impact of advanced competency.

MSN-prepared nurses are uniquely positioned to lead holistic nursing care across a variety of healthcare delivery contexts. Reflect upon the learning that has occurred in Module Eight, and respond to the following:

Describe a scenario involving an individual with complex health concerns. Include the physical, psychosocial, cultural, economic, educational, and environmental factors that influence the health status of the individual in the scenario.

Where is the individual on the health continuum, and why?

How might the MSN-prepared nurse support positive health outcomes through a holistic approach and expanded competency with advanced assessment, pathophysiology and pharmacology?
