1.  Interprofessional practice is possible when members of multiple care professions share goals and collaborate to address complex patient care issues with provision of high-quality clinical care and outcomes in organizations, governance structures, and systems. Read the final report of the Interprofessional Education Collaborative Expert Panel (2011), Core Competencies for Interprofessional Collaborative Practice: Report of an Expert Panel.Download Core Competencies for Interprofessional Collaborative Practice: Report of an Expert Panel.   Discuss how you, as an advanced practice registered nurse, will incorporate each of these competencies into your role by constructing a patient care scenario that enables you to demonstrate and highlight each competency.

2.  Think about a successful healthcare environment where you worked. Discuss in detail one or two habits of system thinking by a team member or leader that you observed in that environment. Do you think these habits improved the care that was delivered to patients and improved efficiency? Why or why not?

1. System thinkers employ several habits that are an essential skill set. Discuss the habits that you employ in your daily work, or the habits you are trying to develop. Discuss why you think these skills are important for you.

1.  Terri is a 83-year-old widow who has been hospitalized and is being prepared for discharge. Terry was hospitalized after suffering a stroke on the right side. She has residual left-sided weakness, difficulty swallowing and communicating, and aspirates easily. The doctor has initiated aspiration precautions and her diet includes pureed foods and nectar thick liquids. While in the hospital, she receives PT, OT, ST, and assistance with her basic hygiene needs. Terri was independent at home, cooked her own meals, drove a car, and did her own laundry and housekeeping. Her closest relative is her 82-year-old brother who suffers from DM and is blind in the right eye. He lives 30 minutes away. How can Terri's APRN direct and manage, or organize and coordinate, Terri’s discharge and who should be part of this process?

2.  What are decision support systems, and what role do they play in the business environment? Give an example of decision support systems used in health care. Discuss the challenges faced by EHR data mining. What is the potential of healthcare data mining? How can it benefit or improve patient outcomes?  Finally, explain why knowledge work and the clinical vocabulary contained in unified medical language systems are important for clinical reasoning and evidence-based practice.

2.  Review the Institute of Medicine report below, Crossing the Quality Chasm: A New Health System for the 21st Century, and determine if the recommendations proposed in this report have changed the scope of practice for contemporary APRNs as it relates to care coordination. Explain your rationale with specific examples.
