Psychosis Case Study Transcript

Chief Complaint: Everly Mann is a 24-year-old African-American female presenting to the clinic with a report of “I see lots of spiders crawling on my body and hands and legs, but no one else can see them. I feel them on my arms and legs. I think they are attracted to the hamburgers.”

History of Present Illness:

Onset: Diagnosed 1 year ago after first psychotic break, voices returned recently as well as tactile and visual hallucinations—spiders on legs and feet. Presents with disheveled appearance, flat affect, tangential speech, reports auditory, visual, and tactile hallucinations daily, some audio hallucinations as well and states her thoughts are being told to others aloud.

Location: Is distracted and poorly performing at work, misses work and is frequently late and unable to concentrate at home with family or alone as well.

Duration: Symptoms are continuous. Auditory hallucinations returned about 6 weeks ago and have drastically increased. Tactile and visual hallucination returned about 5 weeks ago and have gradually increased.

Characteristics: Hears three voices talking to her, “a good voice and a mean voice and a third that is just blubber.” Reporting the “mean voice” making derogatory comments. Denies command voices.

Aggravating: When in public places, is very suspicious that others can hear her thoughts and possibly whispering about her. Has become agitated, chaotic, fearful, and yelled at strangers in public.

Relieving: Can be distracted for a while with music or a movie. Seroquel PO worked adequately for almost a year, then the patient felt she was well enough to stop medication.

Temporal: Worse in the a.m. after waking.

Severity: Outbursts at times when responding to voices or suspicious of others. Whispers constantly to answer voices.

Differentials: List the three most likely differential diagnoses based on her objective findings, with cited rationale.

What is the etiology/pathophysiology associated with each diagnosis?

What is the prevalence of each of these diagnoses?

Develop a plan of care for each of the three differential diagnoses, including the following:

Chief Complaint: Mrs. Lane is a 42-year-old Korean American female who presents stating, “I’m just so sad, I cannot stop crying.” My primary care provider sent me because she says I am depressed and she started me on Lexapro.”

History of Present Illness:

Onset: Past three months worsening of moods, no interest in going out, seeing family, and cooking.

Location: Is distracted at home.

Duration: Depression was diagnosed in primary care two years ago.

Characteristics: Mrs. Lane reports that she has a history of Major Depressive Disorder and Generalized Anxiety Disorder, which is recurrent and was diagnosed by her primary care provider three years ago.

She reports that she was started on Lexapro 10 mg and felt some “better,” until the last couple of months when, she states, “I just feel off.” She has a “depressed” mood and her affect is congruent. She has difficulty falling asleep and staying asleep, as she wakes most nights in the middle of the night.

Her appetite is “the same,” but she has a great deal of trouble concentrating on tasks and has an increase in poor memory. Her memory issues worry her the most, and she inquires if she should be tested for Alzheimers.

She also states she lacks motivation to cook for her family and has not had them over for a meal for three months. Her favorite day of the week used to be Saturdays, when she cooked for her entire family.

Aggravating: With stress and less sleep.

Relieving: Sometimes reading in bed and talking to family during the daytime helps.

Temporal: Worse in the a.m. at first waking and at bedtime before sleep.

Severity: “I feel sad a lot and like everything is in gray shades. I think I’m getting worse.”

Differentials: List the three most likely differential diagnoses based on her objective findings, with cited rationale.

Chief Complaint:Katie Adams is a 19-year-old Caucasian female here and states, “I worry about everything all the time.” “I can’t ever sleep, and I feel anxious and nervous at school and work.” “I cry often for no reason.”

History of Present Illness:

Onset:Following move to out-of-town college, worsening over the past 3 months of being away from home and the stress of transitioning to college.

Location:Heart races, shortness of breath, sweating, headaches, dizzy.

Duration:Daily, lasts 2 to 4 hours then subsides, begins around 10 a.m. before first class starts at 10:30 a.m.

Characteristics: Worsens with attending large size classes, peers and roommates pressure to go out socially and drink, difficulty sleeping, avoids some classes, chest tightness often and shortness of breath.

Aggravating: With increased stress, crowds, when worrying about completing assignments or making good grades.

Relieving: Reading fiction, marijuana, sometimes a beer (1–2), staying in room alone when roommates go out.

Temporal: Worse in morning before classes and worse before bedtime.

Severity: I hate how I feel, it’s horrible.

Chief Complaint:Mr. Bert Colton is an 89-year-old Caucasian male who presents to the clinic with his granddaughter, complaining of “I’m so panicked, I feel like I can’t calm down and I feel like I want to cry.”

History of Present Illness:

Onset:Patient described feeling restless, on edge, fatigued, and irritable since his diagnosis of open-heart surgery one year ago.

Location:Mostly when at home alone.

Duration:Over the past year, he has started to excessively worry more and more about multiple issues.

Characteristics: He endorsed worrying and being anxious most all day, most every day of the week, about what will happen to his daughter when he dies; if she will die a terrible death because of his disease being passed to her, or if she might get breast cancer like his wife died of.

He worries about what if the day comes when she cannot walk or care for herself anymore. What will her church family do without her if she can’t attend biweekly anymore, or the places where she volunteers, like he does. He sees the impact that his disease has and does not want that for his family, too.

For the last six months, in addition to feeling restless, on edge, fatigued, and irritable, he has also had difficulty concentrating and cannot “turn his mind off sometimes” at night to fall asleep.

He has started having trouble walking to the store, which is one block away, for his groceries, paying his bills, going to social gatherings, and going to his neighborhood group meetings, and he has recently decreased his church attendance and volunteer experiences at the local hospital because of his symptoms.

Aggravating: Thinking about dying and leaving family, all the work that needs done to my home, thinking about and missing my wife.

Relieving: Spending time with family.

Temporal: Worse in late evenings and sometimes in middle of the night.

Severity: “I can’t stand this feeling every day, like I need to jump out of my skin.”

Differentials: List the three most likely differential diagnoses based on her objective findings, with cited rationale.
