COMMUNITY HEALTH NEEDS ASSESSMENT 

A community health needs assessment involves a number of steps. As a health educator, you need to be able to complete each step efficiently in order to produce an accurate assessment of the community's health needs. The starting point of a community health needs assessment is to know why you are conducting the assessment and how you are going to use the assessment results. Once you thoroughly understand the purpose of a community health needs assessment, you will be able to conduct the assessment and utilize its results in a more efficient and fruitful manner.

Read this week's resources. Then, choose a community you would like to focus on for this assignment. This can be the community you reside in or a community with which you are familiar. Select a population (e.g., elderly people, teenage mothers) that you want to focus on within the community.

Address the following:

1. Present the purpose of a community health needs assessment:

For this section, clearly articulate your understanding of what a community health needs assessment is and why someone would conduct one. Explain your understanding based on the resources, and apply this information to your chosen community

2. Describe your community and include details about the population within the community:

Using the Understanding and Describing the Community resource from this week’s Learning Resources for guidance, provide a description of the community you selected.

Identify two primary data sources you would use to learn more about the community. Explain why you chose these methods.

3. Consider that you are being asked to complete a community health needs assessment in this community.  What goals would be set?

Explain three overarching goals for a community health needs assessment.

4. Review this week’s Learning Resources on the Social Determinants of Health.

Using your familiarity of the community and resources to support, share your initial concerns relative to the Social Determinants of Health and their interaction with health outcomes in this community. Which Social Determinants (e.g. education quality, health care access) are prominent? Suggest two strategies for integrating a Social Determinants of Health focus in a community health needs assessment.
