Clinical Case Scenario

A 6-year-old previously healthy girl presents to the emergency room with a rash and mouth pain. Her mother took her to see her primary care physician (PCP) 1 week prior with a 2-week history of cough, subjective fever, and malaise. The patient was prescribed amoxicillin for presumed pneumonia. Five days into the antibiotic course, she developed an itchy rash and mouth sores. Her mother reports markedly diminished intake over the previous 24 hours, but no emesis or diarrhea. She has no known medication or food allergies. In the emergency room (ER), her temperature is 102°F (38.9°C), heart rate 122 beats/min, blood pressure 119/73 mm Hg, respiratory rate 18 breaths/min, and oxygen saturation 97% on room air. She is 45 inches tall and weighs 60 pounds. On physical examination, she has bilateral conjunctival injection, ulcers on her tongue and lower lips, and purpuric macules and bullous lesions on her torso and extremities. Chest x-ray reveals an infiltrate in the right lower lobe.

Clinical Documentation Assignment

Clinical portfolios are maintained through CORE ELMS. Students are REQUIRED to enter data into CORE ELMS within seven (7) days of the clinical encounter. Students are expected to update their profile information, clinical schedule, time logs, and field encounters every week.

EXPECTATIONS:

1. It is the student’s responsibility to ensure that his/her preceptor has submitted a midterm and final student performance evaluation via CORE ELMS’ electronic form. Students should check CORE ELMS for the evaluation. If the evaluation has not been submitted then students are to respectfully and politely remind the preceptor to complete evaluation. Both evaluations are REQUIRED.

2. Students are REQUIRED to have a total of 180 hours entered in Hours Tracking. The hours tracking dates must match the dates of the recorded field encounters.

3. Students are REQUIRED to enter a field encounter for every patient seen during the clinical rotation within 7 days of the encounter. Students are REQUIRED to have a total of 10,800 minutes of direct patient (Direct Patient Time = Time with Patient + Consult Time with Preceptor).

4. Students are REQUIRED to have the verification of hours form signed by their preceptor for every clinical day completed. The form must be completed in its entirety (tally the hours). The signed form must be uploaded to hours tracking in CORE ELMS by the student.

You are to attend every practicum experience as scheduled with your preceptor and document ALL patient encounters in CORE ELMS. In order to stay current, it is best to update your CORE ELMS logs after every clinical day. All patient encounters and time logs must be logged within 7 days of accomplishing them or they will not count for your practicum time. There is a minimum of 180 hours and 10,800 minutes of field-encounter entries required for successful completion of this course and these must be entered by end of the course for a passing final grade.
