Week 10 SOAP NOTE

Introduction

Each week you will need to submit one complete SOAP note using the SOAP note template. The SOAP note should be related to the content covered in this week, and the completed note should be submitted to the Submissions Area.

Instructions

Choose an encounter form a patient seen in your clinical setting this week.  Create a SOAP note using the SOAP NOTE TEMPLATE.   This note should be from a problem-focused or complete encounter. This should not be from a follow up visit or a medication refill visit. Remove all patient unique identifiers (medical record numbers, social security numbers, addresses, etc.).

You should also upload a copy of each SOAP note to your CORE Portfolio connected to the correlating patient encounter documentation.

Submission Details:

By the due date assigned, complete at least one SOAP note in the template provided.

Name your SOAP note document SU_NSG6330_W10_SOAP_LastName_FirstName.

Submit your document to the submissions area in BrightSpace AND submit a copy to the correlating patient encounter documentation in CORE.
