Planning for Evaluation

The purpose of this assignment is to continue applying a public health nursing practice model in order to develop another component of a community needs assessment report. You will develop an evaluation plan for your evidence-based intervention in order to determine its effectiveness.

You may choose to include key slides from your population-focused intervention implementation plan from Topic 5 in order to ensure that stakeholders have an understanding of the data collection and intervention planning processes.

Develop a 10-15-slide PowerPoint presentation that includes the following information:

Provide an introductory slide with the overview of the purpose of the presentation.

Briefly summarize the population, health disparity, and the targeted intervention.

Analyze the components of an evaluation plan.

Develop a plan for evaluating the evidence-based intervention.

Identify the criteria (performance measures) selected for your plan of evaluating the evidence-based intervention using S.M.A.R.T. goal formatting. Include short-term and long-term outcomes.

Explain how you will use this evaluation data.

Provide a conclusion slide summarizing the presentation.

Include speaker notes for each slide, detailing how you would present this information to stakeholders.

Support your information with a minimum of two peer-reviewed resources within the last 5 years.
