Assessment Description

The purpose of this assignment is to continue applying a public health nursing practice model by developing an evidence-based intervention to target a community health need.

There are 17 types of interventions that can be used by public health nurses. To address the community health need identified during Topics 3 and 4, develop an intervention plan to target that need.

Create a 10-15-slide PowerPoint presentation with a proposed evidence-based intervention to address the identified community health need. The presentation should include the following:

Introductory slide with the overview of the purpose of the presentation.

Brief summary of information from the Topic 4 assignment (include a brief recap of the population and community health need identified and any health inequities).

Description of the proposed evidence-based intervention targeted to the population and your identified community health need.

Rationale for selecting this evidence-based intervention.

Evidence-based resources to support the selected intervention.

Description of all stakeholders, including the community.

Description of the steps that will be taken to ensure the intervention is culturally sensitive to the community and population.

Conclusion slide summarizing the presentation.

Support your information with a minimum of two peer-reviewed resources within the last 5 years.
