Week 4
your patient has a Personal Health Record...Now what?
Discussion Question
Case Study: A 65-year-old woman was just been diagnosed with Stage 3 non-Hodgkin's lymphoma. She was informed of this diagnosis in her primary care physician's office. She leaves her physician's office and goes home to review all of her tests and lab results with her family. She goes home and logs into her PHR. She is only able to pull up a portion of her test results. She calls her physician's office with this concern. The office staff discussed that she had part of her lab work completed at a lab not connected to the organization, part was completed at the emergency room, and part was completed in the lab that is part of the doctor's office organization.
The above scenario might be a scenario that you have commonly worked with in clinical practice. For many reasons, patients often receive healthcare from multiple organizations that might have different systems.
As you review this scenario, reflect and answer these questions for this discussion.
What are the pros and cons of the situation in the case study?
What safeguards are included in patient portals and PHRs to help patients and healthcare professionals ensure safety?
Do you agree or disagree with the way that a patient obtains Personal Health Records (PHRs)?
What are challenges for patients that do not have access to all of the PHRs? Remember, only portions of the EHRs are typically included in the PHRs.

Week 7
Use of Personal Communication Devices in Patient Care Settings
Discussion Question
A nurse colleague uses his personal cell phone to take a photo of a patient’s wound and then sends a message with the photo to the primary care provider via a text. Please consider the following in your discussion post:
What principles of patient confidentiality might be an issue? Consider legal and ethical.
How might this nurse use a communication device to support safe patient practices?
What would your organization’s policy on 'personal communication device use in a patient care setting' reveal related to this case? (If your organization does not have a written policy, or you are not currently working in an organization, what do you think should be included in such a policy and why?)

Week 2
Course Project Milestone 1
Preparing the Assignment
Select ONE of the Health Conditions or Health Behaviors for the focus of your assignment through the Healthy People 2030 link below:
https://health.gov/healthypeople/objectives-and-data/browse-objectivesLinks to an external site.
Note: It is important to choose your Health Condition or Health Behavior first, then develop a short, but detailed patient scenario (See the video tutorial, assignment directions, and rubric for details about the scenario.) Then explore a mobile health app (mHealth app) that fits the patient and the topic.
Note: You will use the same scenario you develop and same mHealth app you identify here in future assignments (Week 4 and 6).
Requirements and Guidelines
Download the required Milestone 1 TemplateLinks to an external site.. Save it to your computer with the file name: Your Last Name NR361 Milestone 1.docx.
Write your own patient scenario using the Required Assignment Topic.
Make sure to include a Healthy People 2030 Health Condition or Health Behavior in the scenario for a patient with a desire to maintain or achieve good health and prevent illness and who could benefit from an already developed and reliable mHealth app from the internet.
Choose a scholarly peer-reviewed journal article published within the last 5 years and related to the health condition or health behavior in your scenario. This required article citation MUST appear in the Patient Scenario section of the paper and support your chosen health topic. You may use other citations/references in your paper however, these would be optional. The Chamberlain Library is your best place to obtain this article. Cite this in APA 7th edition format in this section of the assignment.
Describe in detail the plan for teaching this patient in the scenario.
Include the nurse's assessment of learning needs and readiness to learn.
Identify a mHealth app that could benefit the patient, considering the teaching/learning needs you identified.
Tips for your internet search for an appropriate mHealth app:
The iPhone App store and Google Marketplace let you search for apps based on key words such as “health” or more specific topics such as “weight loss,” “stress,” and “diabetes”. Look through the descriptions of apps that come up in your results. Does the app address the health concern you have identified? Does it do what you’re looking for with this specific patient? For example, if you’re interested in having your patient track their sleep habits, does the app do this? Please reach out to your instructor if you have questions about a particular mHealth app for this assignment.
Describe the app including the mHealth app name, purpose, intended audience, mobile device(s) upon which it will operate, and where to download or obtain it. Add any other information you believe would be pertinent to this situation.
Include a working link if it is to be downloaded from a website in the area indicated on the template.
Make sure to add a citation for this mHealth app in APA format. The directions and example of how to cite a mobile app are located on page 340 of the APA Manual, 7th edition.
At the end of the Milestone 1 Template there is a References page where you will list your 2 references. One will be the mHealth app reference (also on page 340, APA Manual) and the other will be your scholarly article reference.
You are required to complete the Milestone 1 Template using the productivity tools required by Chamberlain University, which is Microsoft Office Word 2013 (or later version), or Windows and Office 2011 (or later version) for MAC. You must save the file in the ".docx" format. Do NOT save as Word Pad. A later version of the productivity tool includes Office 365, which is available to Chamberlain students for FREE by downloading from the student portal at http://my.chamberlain.edu Links to an external site.
Carefully review the grading rubric criteria for Milestone 1 and type directly on your saved Milestone 1 Template. Be sure to delete the directions under the headings before submitting. The assignment paper should be approximately 2-3 pages, not counting the title page or References page.
The instructor will score Milestone 1 and provide feedback and a clear statement regarding the required assignment topic:
Accepted,
Accepted with revisions needed, OR
Not accepted.
If the Milestone 1 assignment needs revisions or is not accepted, the instructor will indicate if the student should communicate further revisions to the instructor. No additional points will be earned for revisions to Milestone 1, but these revisions will greatly enhance success on later milestones. Milestone 1 must be submitted or re-submitted per instructor feedback before Milestone 2 will be accepted for grading and credit.
The entire project is worth 600 points. Milestone 1 is worth 150 points of this total.
